About This Notice

This Notice of Privacy Practices describes how medical and dental information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.

Rodeo Dental & Orthodontics is required by law to maintain the privacy of your protected health information (PHI), to provide you with this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

How We May Use and Disclose Your Health Information

Rodeo Dental & Orthodontics may use and disclose your protected health information for the following purposes:

Treatment

We may use your health information to provide, coordinate, or manage your dental care. For example, we may share information with specialists, referring dentists, or other providers involved in your care.

Payment

We may use and disclose your health information to bill and collect payment for dental services. This includes sharing information with your insurance company, Medicaid, CHIP, or other third-party payers to obtain prior authorization or verify coverage.

Healthcare Operations

We may use your health information to support our business activities, including quality assessment, staff training, licensing, accreditation, and business planning.

Appointment Reminders & Health-Related Communications

We may contact you to remind you of appointments, provide treatment updates, or inform you of health-related benefits and services that may be of interest to you.

As Required by Law

We will disclose your health information when required to do so by federal, state, or local law, including reporting to public health authorities, responding to court orders, and complying with regulatory agencies.

Public Health & Safety

We may disclose your health information to prevent a serious threat to your health and safety or the health and safety of the public or another person.

Uses and Disclosures Requiring Your Written Authorization

Rodeo Dental will obtain your written authorization before using or disclosing your health information for purposes not described in this notice. This includes:

  • Marketing communications (beyond appointment reminders and health-related services)
  • Sale of your protected health information
  • Most uses of psychotherapy notes, if applicable
  • Any other purposes not permitted or required by HIPAA

You may revoke your authorization at any time by submitting a written request to our Privacy Officer. Revocation will not affect any disclosures made prior to the revocation.

Your Rights Regarding Your Health Information

Under HIPAA, you have the following rights regarding your protected health information:

Right to Access

You have the right to inspect and obtain a copy of your health information, including dental records and billing records. We may charge a reasonable fee for copying costs.

Right to Amend

You have the right to request that we amend your health information if you believe it is incorrect or incomplete. We may deny the request in certain circumstances, and will provide a written explanation if denied.

Right to an Accounting of Disclosures

You have the right to request a list of certain disclosures we have made of your health information. This accounting does not include disclosures made for treatment, payment, or healthcare operations.

Right to Request Restrictions

You have the right to request restrictions on how we use or disclose your health information for treatment, payment, or healthcare operations. We are not required to agree to your request, except in certain circumstances involving payments made in full out of pocket.

Right to Request Confidential Communications

You have the right to request that we communicate with you about your health information in a specific way or at a specific location. For example, you may request that we contact you only at a particular phone number or address.

Right to a Paper Copy

You have the right to obtain a paper copy of this notice at any time, even if you previously agreed to receive it electronically. You may request a copy at any Rodeo Dental office or by contacting our Privacy Officer.

Right to File a Complaint

If you believe your privacy rights have been violated, you may file a complaint with Rodeo Dental or with the U.S. Department of Health and Human Services Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.

Our Responsibilities

Rodeo Dental & Orthodontics is required to:

  • Maintain the privacy of your protected health information
  • Provide you with this notice of our legal duties and privacy practices
  • Follow the terms of the notice currently in effect
  • Notify you in the event of a breach of your unsecured protected health information

We reserve the right to change this notice and make the revised notice effective for health information we already have about you as well as any information we receive in the future. The current notice will be posted at all Rodeo Dental locations and on our website.

Breach Notification

In the event of a breach of your unsecured protected health information, Rodeo Dental will notify you as required by law. Notification will include a description of the breach, the types of information involved, steps you should take to protect yourself, what we are doing to investigate and mitigate the breach, and contact information for follow-up questions.
